29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: a) On or about 10/13/15 on a jobsite at 5440 Camp Rd., next to NYSEG Electric pole # 3344-4-1, Hamburg, NY: An employee was working from an aluminum ladder that had been placed on top of the work platform of a mobile scaffold in order to increase the height of the scaffold, so that he could thread a cable through a pulley attached to the pole that was approx. 18 ft. above the ground. The employee was exposed to a fall of approx. 12 ft. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(c)(3): Supported scaffold poles, legs, post, frames, and/or uprights were not plumb and braced to prevent swaying and displacement: a) On or about 10/13/15 on a jobsite at 5440 Camp Rd., next to NYSEG Electric pole # 3344-4-1, Hamburg, NY: An employee was working from an aluminum ladder that had been placed on top of the work platform of a mobile scaffold in order to increase the height of the scaffold, so that he could thread a cable through a pulley attached to the pole that was approx. 18 ft. above the ground. The scaffold was not plumb and was leaning against the pole. Additionally, the mobile scaffold had only 3 wheels,and two of those wheels lacked a mudsill. The fourth leg of the scaffold had a peg-leg setting on a mud sill. NO ABATEMENT CERTIFICATION REQUIRED
