29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  A) On or about October 13, 2015 employees were working off the top section of a two tear metal tubular welded framed scaffold system that was not fully planked between the uprights of the scaffold, exposing employees to a fall in excess of 10 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  A) On or About October 13, 2015 Employees working on the top section of the scaffold to fill the concrete CMU walls did not have a ladder available to access the top sections of the scaffold and were climbing the cross braces as a means of access, exposing the employees to fall hazards in excess of 6 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  A). On or about October 13, 2015 employees were working at the top section of a tubular welded scaffold system, to pump concrete into the empty cells of the CMU Wall, and were not protected by guardrails or personal fall protection, exposing them to falls to the surface below in excess of 10 feet.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  (a) On or about October 13, 2015 Employees working along the top of an 8" wide CMU concrete wall to pump concrete into the empty CMU block cells, were not protected from impalement hazards because none of the reinforcing steel rods were capped or guarded.

On or about October 13, 2015 Employees were pumping fresh concrete into the CMU Concrete block wall and were not protected by wearing proper head and face equipment exposing employees to struck by hazards of wet cement, debris and dirt to the face and eyes.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Employees are filling empty CMU concrete block cells with pumped concrete, and are walking along the top of the 8" wide CMU Concrete wall and are not protected by any means of personal fall protection, guardrails or safety nets, Exposing employees to a fall in excess of 10 feet.
