29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:   a) On or about October 13, 2015; rear of building; employees were using a ramp to enter the building which was not properly guarded exposing employees to a 7 foot fall to the lower level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about October 13, 2015; roof area; an employee was in the process of framing an 8:12 pitch residential roof without fall protection in place to protect him from falling an estimated 35 feet to ground level.    b) On or about October 13, 2015; second floor; employees were exposed to a 10 foot fall while working near floor openings.    c) On or about October 13, 2015; second floor; employees were exposed to an estimated 20 foot fall to ground level while working near the wall opening.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

Abatement certification and documentation required within 10 days after abatement date.
