29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about October 8, 2015, at 4711 Bayou Black Drive, Gibson, LA, the employer did not ensure that precautions were taken to eliminate or control sources of ignition in locations where flammable vapors may be present. Employees were conducting preheat setup on a pipeline vessel known as a slug catcher and grinding spots and popping pins on the shell of the pipe that contained flammable hydrocarbon materials.

29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which where subjected to heat, might produce flammable or toxic vapors: On or about October 8, 2015, at 4711 Bayou Black Drive, Gibson, LA, the employer did not ensure that a gas pipe line vessel known as a slug catcher had been cleaned so thoroughly as to make absolutely certain there were no flammable materials (hydrocarbon sludge) present or any substance. Employees were grinding spots and popping pins on the shell of the pipe to setup preheat equipment.
