On or about October 14, 2015 employees working on the 2nd floor of a residential project along the stairwell and the balcony edge were not protected by any means of personal fall protection guardrails or safety nets, exposing employees to a fall hazard in excess of 10 feet.

On or about October 14, 2015 employees were using a 6 foot portable "A" frame ladder to gain access to the 2nd floor top plate of the exterior wall to secure the installed roof trusses to the top plate and were not protected from falling to the surface below because the ladder did not extend at least 3 feet above the upper surface, exposing employees to a 9-foot fall hazard.
