29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about October 19, 2015, an employee was exposed to the hazard of being struck by flying projectiles, while using a pneumatic nail gun to install roof decking without the use of eye protection.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   On or about October 19, 2015, employees were exposed to a fall of greater than 6 feet to the ground level below, while framing near a leading edge on a residential construction without the use of fall protection equipment or an alternative fall protection plan.
