29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     On or about October 15, 2015 at a job site located at 134 Lincoln Ave., Youngstown, Ohio 44503, during construction activities including but not limited to masonry installation, the employer did not establish and implement a written respiratory protection program. Employees were mixing Type 1/11 masonry mud that contained Portland cement.

On or about October 15, 2015, while performing masonry activities on a jobsite located at 134 Lincoln Ave., Youngstown, Ohio 44503, the employer failed to protect employees from potential eye injuries while using a concrete cut off saw to cut bricks by requiring the employee to wear safety glasses.

On or about October 15, 2015, while performing masonry activities on a jobsite located at 134 Lincoln Ave., Youngstown, Ohio 44503, the employer failed to protect employees from a fall of approximately 25 feet when a top and a mid-rail were not installed on the end of the masonry scaffold.

On or about October 15, 2015, while performing masonry activities on a jobsite located at 134 Lincoln Ave., Youngstown, Ohio 44503, the employer failed to protect employees from a fall of approximately 5 feet when the ladder used to access the roof did not extend at least 36 inches past its landing area.
