29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: At a construction site, employees working from an unsecured manmade platform that was not designed or approved by the manufacturer were elevated by a JCB load-all tele-handler forklift exposing the employees to fall hazards up to approximately 15 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: At a construction site, employees worked from an unsecured manmade platform that was not attached/secured to the forklift while it was elevated exposing the employees to fall hazards up to approximately 15 feet.
