29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  Highline Broadband project site located along North Bowen Ave, Oshkosh, WI: Prior to 10/14/15, the employer had not developed programs to communicate workplace hazards to the employees so they could identify and avoid, or eliminate unsafe conditions associated with installing communications cable on existing poles.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  Highline Broadband project site located along N. Bowen Ave. Oshkosh, WI: On 10/14/15, an employee working from an Altec TA40 boomlift was not protected from falls by a full body harness and lanyard.

Highline Broadband project site located along N. Bowen Ave. Oshkosh, WI: Prior to 10/14/15, the employees had not been trained to recognize and avoid or eliminate hazards associated with working from boomlifts, to include fall and electrical hazards.
