Location: c/o Broadway and Bloomfield Ave., Newark, NJ.   a) The attachment plug installed on the power cord, being used to energize the Bosch hammer drill, was missing the ground pin;  on or about 10/15/15.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: c/o Broadway and Bloomfield Ave., Newark, NJ.   a) Employer did not protect employees from fall hazards while working near the edge of the elevator shaft's temporary wood deck and/or protection.  The employees were exposed to potential fall hazards of up to approximately 13 ft. to the floor below; on or about 10/15/15.

Employees were exposed to a potential fall of approximately 10 ft to the floor below when walking near the holes towards their work area (elevator shaft); on or about 10/15/15.
