29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Location: Building 16, 72 Forest Glen Circle, Middletown, CT 06457 Each employee using portable ladders was not provided with a ladder training program to instruct each employee in the recognition of hazards associated with ladders and the means/procedures to minimize and or eliminate those hazards. On or about October 15, 2015, an employee fell approximately 20 feet from a non-self supported fiberglass extension ladder while power washing a three story apartment building.
