OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from a metal dust fire or explosion hazard: Reading Alloys, Building 11, North outside of Building - Employees were exposed to fire and explosion hazards from the dust Collector/Baghouse 18, which collected combustible metal dust from process equipment inside building. The baghouse dry filter media in baghouse 18 were not antistatic in nature nor were they grounded while in use to prevent dust ignition from a static charge and/or spark. In addition, the placement of the isolation flap valve was not in position to prevent the energy of the dust deflagration from reentering the building to prevent damage to the process equipment on or about October 8, 2015.
