Located at 7540 County Road 201 in Mount Hope, Ohio: The employer did not conduct a job hazard assessment for the operation of high temperature nylon slotter machines. Employees were exposed to a serious burn hazard while handling the nylon materials and operating the machine.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. Located in the Arborist Department: On or about October 15, 2015 and at times prior, employees were using an Master Mechanic electric drill press, model ZJ4116QC, in order to drill holes through nylon belts for copper riveting. The unguarded rotating chuck of the drill press exposed employees to a caught-in hazard.   b. Located in the Arborist Department: On or about October 15, 2015 and at times prior, employees were using an Master Mechanic electric drill press, model MM8050, in order to drill holes through nylon belts for copper riveting. The unguarded rotating chuck of the drill press exposed employees to a caught-in hazard.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a.     Located in the Rope area of the Arborist Department: On or about October 2, 2015 and at times prior, employees were operating a pneumatic clamping machine to press clamps in order to secure metal snap hooks on nylon ropes. The point of operation was unguarded, exposing employees to an amputation hazard.     b.     Located in the Arborist Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Weaver tag number 265, to secure buckles on nylon arborist work belts. The point of operation was unguarded, exposing employees to an amputation hazard.     c.     Located in the Custom Manufacturing area of the Nylon Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by Milford Rivet and Machine Company, Model 256, Serial 2386, Weaver tag number 466, to rivet small leather and nylon pieces. The point of operation was unguarded, exposing employees to an amputation hazard.     d.     Located in the Pouches, Scabbards, Cinches cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Weaver tag number 60, to rivet leather materials together for pouches, scabbards and cinches. The point of operation was unguarded, exposing employees to an amputation hazard.     e.     Located in the Union Lock cell of the Leather Department: : On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by Chicago Rivet & Machine Company, Model 912-5661, Serial 36918, Weaver tag number 235, in order to secure buckles on leather weight lifting belts. The point of operation was unguarded, exposing employees to an amputation hazard.   f.      Located in the Union Lot cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Weaver tag number 386, to rivet leather materials for dog leashes. The point of operation was unguarded, exposing employees to an amputation hazard.     g.     Located in the 867 cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Weaver tag letter A, to rivet leather materials for dog collars and spur straps. The point of operation was unguarded, exposing employees to an amputation hazard.     h.     Located in the 867 cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Weaver tag number 264, to rivet leather materials for dog collars and spur straps. The point of operation was unguarded, exposing employees to an amputation hazard.     i.      Located in the 205 cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by Milford Rivet and Machine Company, Model 256, Serial 3235, Weaver tag number 179, to rivet leather materials for head stalls. The point of operation was unguarded, exposing employees to an amputation hazard.     j.      Located in the Paint & Spot cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by ACME Rivet and Machine Corporation, Model 220r, Serial P13803, Weaver tag number 34, to rivet leather materials for head stalls and spur straps. The point of operation was unguarded, exposing employees to an amputation hazard.     k.     Located in the Basic Tack cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a rivet machine manufactured by Chicago Rivet & Machine Company, Model 912-6364, Serial 36902, Weaver tag number 234, to rivet leather materials for head stalls. The point of operation was unguarded, exposing employees to an amputation hazard.     l.      Located in the Paint & Spot cell of the Leather Department: On or about October 15, 2015 and at times prior, employees were operating a spot setter manufactured by Standard Rivet Company, Serial 104, Weaver tag number 151, to place decorative materials on leather belts. The point of operation was unguarded, exposing employees to an amputation hazard.

Located in the Arborist Department at 7540 County Road 201 in Mount Hope, Ohio: On or about October 2, 2015, two maintenance employees had an occupational exposure to blood while cleaning and sanitizing the concrete floor and a pneumatic clamp machine after an amputation incident occurred. Employees involved in the task were not offered a Hepatitis B vaccination post exposure.
