(b) On or about October 16, 2015, during the installation of vinyl siding on a newly constructed residential home located at Homestead 6 Harvester Dr., Copley, Ohio, 44321, the employer failed to protect employees from a fall of approximately 24 feet when it failed to require employees to use fall protection. (c) On or about October 16, 2015, during the installation of vinyl siding on a newly constructed residential home located on Homestead 6 Harvester Dr., Copley, Ohio 44321, the employer failed to protect employees from a fall of approximately 24 feet when it exceeded the height requirement of 20 feet on a ladder jack scaffold. (d) On or about October 16, 2015, during the installation of vinyl siding on a newly constructed residential home located on Homestead 6 Harvester Dr., Copley, Ohio 44321, the employer failed to protect employees from a fall of approximately 24 feet when it bridged two working levels of a ladder jack scaffold together.
