29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. At a commercial construction site, employees performing brick masonry activities from a tubular welded frame scaffold were not provided with a ladder for safe access and egress from the different working levels on the scaffold, exposing employees to fall hazards of approximately 14 feet to the ground below.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur. At a commercial construction site, employees performing brick masonry activities from a tubular welded frame scaffold were exposed to a possible scaffold collapse and a fall hazard of approximately 14 feet to the ground below where the employer used a forklift to position a bucket of mortar on the top level and the scaffold components were not locked together by locking pins.
