29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net system, personal fall arrest system:    a. On or about October 7, 2015, roof area, employees of Commercial Roofing Company, LLC were installing a TPO roof system and were not protected from falling 27 feet 6-inches by the use of a guardrail system, safety net system, personal fall arrest system or by any other type of fall protection method.

On or about October 14, 2015, employees of White's Plumbing, Inc. were using a 6-foot Werner step ladder in the closed position.  The ladder was leaned against the wall of a 3-buck high scaffold system for employees to access the roof.  Employees were being exposed to fall hazards.
