On or about October 15, 2015 at jobsite located at 5031 Flournoy Lucas Rd, Shreveport LA, the employer allowed employees to operate Dewalt Grinders (Model #D28402), without the adequate guard on during operation. a. Dewalt grinder (Model #D28402) at the North end of the warehouse did not have guard attachment connected. b. Dewalt grinder (Model #D28402) at the South end of the warehouse did not have guard attachment connected.
