29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use. On or about October 15, 2015 at jobsite located at 505 Aero Dr, Shreveport, LA, the employer throughout the warehouse establishment, had pressurized cylinder tanks without valve covering caps. a. On the south side of the warehouse in the welding/fabrication area, one (1) compressed Argon cylinder was secured to a frame without a valve cover cap. b. At the center of the warehouse, one (1) Compressed Argon pressurized cylinder was secured to a steel beam without a valve cover cap. c. In the storage area for cylinders, the employer had two (2) full Acetylene pressurized cylinders without a valve cover cap on it. d. In the empty storage area for cylinders, the employer had four (4) Oxygen pressurized cylinders without a valve cover cap on it. e. In the storage area for cylinders, the employer had two (2), Nitrogen pressurized cylinders without a valve cover cap on it.
