Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes. 2901 Scott Futrell: On or about October 8, 2015, Air Handling Unit #6 was not equipped with a guard on the left interior side to protect employees from amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Charlotte P & DC: On or about October 15, 2015, employer did not ensure periodic inspections of the energy control procedures were performed at least annually within the facility including, but not limited to: Chillers, Boilers, AHUs, Air Compressors, Air Dryers, Hot Water Pumps, Chill Water Pumps, Condenser Water Pumps, Cooling Towers, Battery Chargers and Battery Charging Machines. Employees were exposed to the hazards associated with unexpected re-energization of equipment. The U. S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 917645, Citation 2, Item 3 and was affirmed as a final order on 9/1/2014, with respect to a workplace located at 2001 Mattis Ave, Champaign, IL 61821. The U. S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number280116, Citation1, Item 1b and was affirmed as a final order on 9/7/2014, with respect to a workplace located at 3655 E. Fountain Blvd, Colorado Springs, CO 80910.
