29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about October 19, 2015, the employer failed to ensure the employees working on the roof were protected from falling through the unprotected skylights. Employees were working around unguarded skylights with no fall protection. Employees exposed to fall hazard greater than six feet to the next level. Employees also exposed to a laceration hazard from the glass.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about October 19, 2015, the employer failed to ensure the employees working on the roof were protected from a fall hazard. Employees were installing a metal roof and were exposed to fall hazard of approximately nineteen feet to the lower level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about October 15, 2015, at the site, the employer failed to ensure the each employee that was installing the metal roof were trained to recognize the hazard. The employees were exposed to a fall hazard or approximately nineteen feet.
