29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) Charles Washington Building:  On or about October 14, 2015 and times prior, employees were preparing a window opening for window installation while working from a frame scaffold that was not fully planked exposing employees to falls of up to approximately 20ft. to the ground below.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:    (a) Charles Washington Building:  On or about October 14, 2015 and times prior, employees were exposed to falls of up to 20ft. while working from a frame scaffold that did not have a fall protection system in place.

(a) Charles Washington Building:  On or about October 14, 2015 and times prior, employees required to work from a frame scaffold to prepare window openings for window installation were not trained to recognize hazards associated with working from the scaffold and working from heights.
