29 CFR 1910.29(e)(1): "Design." Units shall be designed for the use intended and shall comply with the requirements of paragraph (a) of this section. a. On or about October 19, 2015, in the vault, employees were required to use a mobile platform that was missing the rubber stoppers on the end of the ladder's feet, therefore the ladder could not lock into place. Employees were exposed to fall hazards.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. a. On or about October 19, 2015, in the facility, employees were required to operate forklifts and Wave industrial vehicles on a daily basis. The forklift drivers were not retrained after three years as required by the standard.
