29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Main roof area: On October 15, 2015 employees wearing fall protection harnesses were not attached to the fall protection system that was in place.  The employees were performing re-roofing activities on a 6 in 12 pitched roof.     b) Side roof area: On October 15, 2015 an employee wearing a fall protection harness was not attached to an anchorage point while performing re-roofing activities.  The employee was performing re-roofing activities on a 4 in 12 pitched area of the roof.   c) Roof over garage: On October 15, 2015 a single anchorage point used as part of a fall protection system, was only halfway installed.  Two employees performing re-roofing activities on a 9 in 12 pitched roof, were attached to the anchorage point.   Note) Certification of corrective action IS required for this item.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:  a) Over garage: On October 15, 2015 two employees were attached to a single temper anchor as part of a personal fall protection system.  Only one side of the anchor was attached to the roof.    Note) Certification of corrective action IS required for this item.
