An employee was not provided annual audiograms for 2014 and 2015 while working on the Gel Deck as  an operator. Employee(s) were exposed to continuous noise levels equivalent to 86.68% (88.9 dbA) of the allowable dose of 50%, or 85 dbA for an 8 hour time-weighted average, and which is 1.7 times the allowable exposure limit. Sampling was performed for 475 minutes on 11/18/2015, with the unsampled portion of time considered to represent zero for the exposure calculation.    Violation occurred on or about 11/18/2015.
