29 CFR 1910.95(g)(6): "Annual audiogram." At least annually after obtaining the baseline audiogram, the employer shall obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels.    a) Facility    The employer did not obtain annual audiograms for all employee(s) at or above the action level of 85 dbA as an 8 hour time weighted average.       An employee was not provided annual audiograms for 2014 and 2015 while working on the Gel Deck as  an operator. Employee(s) were exposed to continuous noise levels equivalent to 86.68% (88.9 dbA) of the allowable dose of 50%, or 85 dbA for an 8 hour time-weighted average, and which is 1.7 times the allowable exposure limit. Sampling was performed for 475 minutes on 11/18/2015, with the unsampled portion of time considered to represent zero for the exposure calculation.    Violation occurred on or about 11/18/2015.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:      a. 141 Southside Ave., Bridgewater, NJ -      Nitta Casings, Inc./ NJ site: The Employer did not provide training  for employee(s) exposed to noise above 85dbA. Employee(s) were exposed to continuous noise levels equivalent to 86.68%  (88.9 dbA) of the allowable dose of 50%, or 85 dBA for an 8-hour TWA. The levels are above the 8-hour TWA of 85dBA over 475 minutes. Zero exposure was assumed for all unsampled time periods.     Violation observed on or about 11/18/2015.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h):    a. 141 Southside Avenue, Bridgewater, NJ      The employer did not maintain and implement a written hazard communication program.  The written Hazard Communication Program was not updated to include Global Harmonization elements, updated Safety Data Sheet master library, and methods the employer would use to inform employees of non-routine hazards. Products include but not limited to  Ammonia, Sodium Hypochlorite, Compressed Oxygen, Acetylene, and Compressed Chlorine.        Violation occurred on or about October 6, 2015.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a. 141 Southside Avenue, Bridgewater, NJ      Employees were not provided effective training on hazardous chemicals used daily in their work area.  Employees engaged in the production of collagen casings used hazardous chemicals, which included, but were not limited to, Ammonia, Compressed Oxygen, Compressed Chlorine, Sodium Hypochlorite on a daily basis and were not provided training on their hazards.       Violation occurred on or about October 6, 2015
