29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Front and backside of residential structure: On October 15, 2015 and at times prior thereto employees performed framing activities on a 7 in 12 pitched roof, along with other lower pitched areas of the roof without a fall protection system in place.    b) Second floor: On October 15, 2015 and at times prior thereto employees performed framing activities near an unprotected window opening on the second floor level.     c) Second floor: On October 15, 2015 and at times prior thereto a handrail and stair rail system was not in place on the stairway that employees used for access to the second floor.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  a) Jobsite: On October 15, 2015 and at times prior thereto employees did not recognize or acknowledge the hazard of performing framing activities on a pitched roof above 6 feet without a fall protection system in place.  Nor did they take action to correct or minimize the hazard.    b) Jobsite: On October 15, 2015 and at times prior thereto employees did not recognize or acknowledge the hazard of performing framing activities near an unprotected window opening above 6 feet without a guardrail system in place.  Nor did they take action to correct or minimize the hazard   c) Jobsite: On October 15, 2015 and at times prior thereto employees did not recognize or acknowledge the hazard of using a stairway without a handrail or stair rail system in place.  Nor did they take action to correct or minimize the hazard.   Note) Certification of corrective action IS required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Exterior of residential jobsite: On October 15, 2015 an extension ladder used by employees for access to the lower levels of the roof did not extend 3 feet above the eave of the roof.
