29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:         (a)  LEI Companies, Inc. at 9191 Grant Street, Thornton, CO 80229: On or before October 9, 2015, the employer did not ensure that employees working on the electric power circuit in the MRI electrical room at North Suburban Medical Center were protected from electric shock by de-energizing the circuit before work begun. The employees were making a tie-in to a neutral conductor that was being shared with at least one other live circuit. This condition resulted in both employees receiving an electric shock of 277 V, phase to ground and one of the employees being hospitalized after the incident.

29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken:      (a)  LEI Companies, Inc. at 9191 Grant Street, Thornton, CO 80229: On or before October 9, 2015, the employer did not ensure employees working on the electric power circuit in the MRI electrical room at North Suburban Medical Center were advised of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken before work begun. 29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):      (a)  LEI Companies, Inc. at 9191 Grant Street, Thornton, CO 80229: On or before October 9, 2015, the employer did not ensure that a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Two employees engaged in electrical work activities were exposed to shock hazards because they did not adequately deenergize and test for energized circuits while in the MRI electrical room at North Suburban Medical Center. The employees were making a tie-in to a neutral conductor that was being shared with at least one other live circuit. This condition resulted in both employees receiving an electric shock of 277 V, phase to ground and one of the employees being hospitalized after the incident.
