29 CFR 1926.403(b)(1): The employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  At the jobsite located at 16209 Wedd Street in Overland Park, Kansas:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.   110 VAC energized conductors were exposed on the female plug end of the extension cord providing power to the Makita circular saw.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the jobsite located at 16209 Wedd Street in Overland Park, Kansas:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.   Strain relief was not provided on the female plug end of the extension cord providing 110 VAC power to the Makita circular saw.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the jobsite located at 16209 Wedd Street in Overland Park, Kansas: Employees engaged in residential framing and decking activities from a 3:12 pitch roof were exposed to fall hazards of approximately 13 feet from the eave of the residence to the ground below in that fall protection was not provided/utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 16209 Wedd Street in Overland Park, Kansas:  Employees engaged in residential framing activities were exposed to fall hazards of approximately 13 feet from the eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the residence/structure.
