29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:     Mill Coatings project site located at US51 at Falcon Dr. Brokaw, WI: On 10/16/15, an employee was exposed to falling while working from a pile of scaffold planks supported by a rough terrain forklift.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection:     Mill Coatings Inc. project site located at US51 at Falcon Dr. Brokaw, WI: On 10/16/15, an employee was exposed to falls while working from the load supported by a rough terrain forklift when:  A)    his personal fall arrest system lanyard was wrapped around the backrest of the forklift and hooked onto itself;    B)    when his personal fall arrest system lanyard was wrapped around the backrest of the forklift which had sharp edges that could cut or abrade the lanyard in a fall;    C)    when his personal fall arrest system lanyard was wrapped around the backrest of the forklift which is not independent of the supporting structure;    D)    when his personal fall arrest system lanyard was wrapped around the backrest of the forklift near his foot level, allowing him to fall greater than 6;    E)     and a second employee was exposed to serious injury in the event of a fall while wearing his harness under his coat with the lanyard coming out at his waist which would cause the employee to fall upside down.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:       Mill Coatings Inc. project site located at US51 at Falcon Dr. Brokaw, WI: On 10/16/15, employees were exposed to fall hazards while erecting and working from a suspended scaffold that had not been designed by a qualified person or erected in accordance with written instructions or plans, in that:  A) the capacity of the components used in the construction of the suspended scaffold were not known;    B) the scaffold system had not been evaluated by a qualified person for compliance with the scaffold Standards;    C) the parapet brackets were not tied back or provided with other means to prevent inadvertent movement;    D) scaffold platforms were not installed level when the profile of the girders changed at the center bridge pier;    E) scaffold platforms are not secured to the trusses to prevent uplift from wind;    F) and the scaffold system is not assembled in accordance with any written instruction or diagrams.
