29 CFR 1910.22(b)(1): 29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. a) Facility floor: The aisles and travelways around the small box area are not maintained free of obstructions in that the doors when left open reduce the width of the aisles and expose employees to struck-by hazards from the concrete forms. b) Facility floor: The aisles and travelways around the big box area are not maintained free of obstructions in that the doors when left open reduce the width of the aisles and expose employees to struck-by hazards from the concrete forms. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

On or about October 9, 2015, the quadruple sling located in the "Big Box Area" and used with the overhead crane did not have identification stating size, grade, rated capacity, and reach. b) Facility floor: An alloy steel sling did not have to have identification stating size, grade, rated capacity and reach. On or about October 9, 2015, the quadruple sling located in the "Bigger Box Area" and used with the forklift did not have identification stating size, grade, rated capacity, and reach. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
