Bryan Packaging, Inc. - Bryan, Ohio: On or about October 20, 2015, employees were exposed to the in-going nip point of rollers while feeding cardboard into the feed end of the General Corrugated Machinery (s/n HDL 9062) gluer/taper 160 machine.       d. An employee sustained an amputation injury after inadvertently contacting the unguarded rotating part.    b. Bryan Packaging, Inc. - Bryan, Ohio: On or about October 20, 2015, the employer did not ensure that the rotating parts, used to move the conveyor belts for the stacker on the Langston 66-inch Press 125 were guarded.  Employees realigning and gathering cardboard were exposed to rotating parts of the delivery rollers.    c. Bryan Packaging, Inc. - Bryan, Ohio: On or about October 20, 2015, the employer did not ensure that the rotating shaft, associated with the Langston 66-inch press motor,  located near operating controls on the feed side of the press, was enclosed to protect employees from the rotating part hazard.       d. Bryan Packaging, Inc. - Bryan, Ohio: On or about October 20, 2015, the employer did not ensure that rotating parts of the Greene Line stacker machine (s/n 2N1464) were guarded.
