29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:   (a) 7432 Lohmeyer Avenue, Maplewood:  Employees who used a Werner aluminum extension ladder to gain access to a roof were nor provided required training to prevent using a ladder which extended approximately 15 inches above the landing surface and carrying roofing material on one hand while climbing the ladder.
