29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Facility, Weld Shop Area: On or about October 20, 2015, an employee was operating an 8 foot mechanical press brake and he was not protected from having any part of his body in the danger zone during the operating cycle of the mechanical press brake. In addition, the foot pedal (which activated the machine) was not guarded against accidental contact and activation.
