On or about October 18, 2015 an employee fell from the top working level of the mobile scaffold while using manual force at the top of the mobile scaffold to move the scaffold. The castors were not locked while the employee worked from the scaffold so the employee did not have to come down from the scaffold to unlock the castors before moving the mobile scaffold.

On or about October 18, 2015 an employee fell from the top working level of the mobile scaffold while using manual force at the top of the mobile scaffold to move the scaffold. The scaffold did not remain square while being moved contributing to a scaffold failure causing the employee to fall.
