29 CFR 1910.178(m)(6): A safe distance shall be maintained from the edge of ramps or platforms while on any elevated dock, or platform or freight car. Trucks shall not be used for opening or closing freight doors. On or about October 19, 2015, at the worksite located at 8606 Mabelvale Pike, Little Rock, Arkansas, an employee did not use his seat belt at some point in time while operating a forklift that was equipped with a seat belt on and/or near the edge of an elevated ramp, platform, or dock, which exposed the employee to a crushed-by hazard.
