29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such hole. On or about October 17, 2015, and at times prior to, at the Cheniere Plant, Bechtel Oil, Gas & Chemicals Construction Services failed to ensure that employees tied off with their fall arrest system to a suitable anchorage point while working inside a guardrail system that was protecting a 40" pipe penetration.
