29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 15, 2015, the employer failed to develop machine specific procedures for the control of hazardous energies. Employees perform servicing and maintenance activities on equipment including, but not limited to, punch presses, feedline equipment, Hydraulic presses, robotic welders, cranes, air compressors, and CNC equipment. Multiple energy sources include: pneumatic, hydraulic, mechanical, gravity, and thermal.

29 CFR 1910.217(c)(3)(vii)(d): Two hand controls used with mechanical power press(es) were not fixed in position so that only a supervisor or safety engineer was capable of relocating the controls: On or about October 12, 2015, the employer failed to ensure that two hand control pedestals were fixed in position on the Clearing mechanical power press (model s2-600-112,54, company press #201). The pedestals were easily moveable by employees.

Employees performed a die change placing themselves in the point of operation and other hazardous areas including pinch points.
