29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  At the commercial jobsite, employees performing roofing activities were not protected from a fall hazard where:  a) On the south side of the building on the lower roofs above the "exit", employees installing metal flashing were not protected from a fall hazard of approximately 14 feet to the concrete below; and  b) On the west side of the building, employees washing the roof and performing punch list activities were not protected from a fall hazard of approximately 20 feet to the concrete below.  The R & B Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314273855, citation number 1, item number 2 and was affirmed as a final order on 11-16-2010 with a final abatement date of 08-31-2010 with respect to a workplace located at 1900 Green Oaks Road, Fort Worth, TX 76116.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    At the commercial jobsite, employees performing roofing activities were gaining access to the roof by a means of a ladder that did not extend at least 3 feet above the upper landing, exposing employees to a fall hazard of approximately 12 feet to the concrete below.  The R & B Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 314273855, citation number 1, item number 3 and was affirmed as a final order on 11-16-2010 with a final abatement date of 08-31-2010 with respect to a workplace located at 1900 Green Oaks Road, Fort Worth, TX 76116.
