(a) Roof of the house at 1860 Tululah Trail, Smyrna, GA: An employee was exposed to fall hazards while improperly using a harness and carabineers as a positioning belt with a rope grab located at the wearer's waist. The employees were on a 8:12 pitched roof at a height of approximately 22 feet from the eave to the lower level.
