29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  a)    Joint Expeditionary Base Fort Story, BEQ Renovation, Buildings B860-862: On or about 10/20/2015, two (2) 100-foot extension cords plugged into two (2) 115 volts wall receptacles used to supply power to portable floodlights were not protected with a ground fault circuit interrupter, as a result employees were exposed to possible burns resulting from electrical shock.   No abatement certification or documentation required.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a)    Joint Expeditionary Base Fort Story, BEQ Renovation, Buildings B860-862: On or about 10/20/2015, the path to ground on the 100-foot orange extension cord used to supply power to a portable floodlight was not permanent and continuous in that the ground prong was missing from the male attachment plug thus exposing employee to a possible shock hazard.     No abatement certification or documentation required.
