29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a)  F and L Construction LLC. on or about October 21, 2015, at the address of 4310 Albion Rd. Monclova, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards common in the roofing industries such as falls, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  F and L Construction LLC. on or about October 21, 2015, at the address of 4310 Albion Rd. Monclova, OH: The employer failed to ensure employees were protected from falls while installing new roofing material atop a residential structure.  Employees working in this condition were exposed up to a 10 ft. fall to the ground below.

Monclova, OH: The employer did not ensure employees were wearing appropriate eye protection such as, safety glasses while using a pneumatic nail gun to fasten roofing material. The employees using these tools were exposed to potential eye injuries from flying objects.
