29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    On or about October 21, 2015, and times prior thereto, the employer did not ensure employees operating heavy machinery at a road construction work site were trained to recognize and avoid unsafe conditions while working around electrical power lines, exposing the employees to electrical shock and electrocution hazards.

29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into pysical or electrical contact with the electric power circuit:    On or about October 21, 2015, and at times prior thereto, the employer did not ascertain whether any part of an overhead power line was so located that the raising of the bed of a dump truck while connected to a paver would come into physical or electrical contact, exposing employees operating heavy machinery under the energized power lines to electrical shock and electrocution hazards.

29 CFR 1926.600(a)(6)(ii): For lines rated 50kV or above, a minimum of 10 feet plus 0.4 inch for each 1kV over 50 kV was not maintained between the energized line and the equipment:    On or about October 21, 2015, and at times prior thereto, the employer did not ensure his employees maintained a clearance distance of at least 10 feet 8 inches between the overhead power lines and the heavy machinery they were operating, exposing the employees to electrical shock and electrocution hazards.
