29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: a) At the facility, on or about 10/21/15: The written respiratory protection program provided by New York State Department of Labor, OSHA Consultative Services, was not customized for the employer's work place by filling in the blanks as required. Broome County Automotive Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c) which was contained in OSHA inspection number 962587, citation 1, item 1a and was affirmed by final order on 7/14/14.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: a) At the facility, on or about 10/21/15: An estimate of employee exposure was not performed for exposure to methylene bisphenyl isocyanate during bed liner spray application operations. The employee performing the operation was wearing a supplied air respirators with hood.
