29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: At a commercial construction site, an employee performing masonry work using a STIHL Cutquik saw on the northwest side of the building was not provided with eye and face protection exposing the employee to eye and face injuries.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. At a commercial construction site, the employer did not ensure an employee performing masonry work from a scaffold platform that was elevated by a Genie GTH-844 Rough Terrain Forklift was provided with fall protection exposing the employee to a fall hazard of approximately 16 feet to the ground below.
