29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  At a commercial construction site, the employer did not ensure an employee performing masonry work from a scaffold platform that was elevated by a Genie GTH-844 Rough Terrain Forklift was provided with fall protection exposing the employee to a fall hazard of approximately 16 feet to the ground below.
