29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about October 19, 2015, and at times prior thereto, workers performing roofing operations were not protected from falling through the unprotected skylights. Workers exposed to a fall hazard greater than six feet to the next level and the hazard of being cut by broken glass.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 19, 2015, and at times prior thereto, workers were installing a metal roof on the residential home under construction without fall protection. Workers were exposed to fall hazard up to approximately nineteen feet to the ground below.
