29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected: At this commercial construction site, employees were working from a scissor lift welding a metal canopy structure. Employees at this location were exposed to electrocution hazards as welding leads provided were not maintained in a safe manner. The welding leads utilized had several sections with exposed wiring and sections covered with black electrical tape at 25 inches from the electrode.

Employees at this location were exposed to fire hazards as a fire extinguisher provided # MV-067671 was not maintained and ready for use. The extinguisher indicated "recharge".

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). At this commercial construction site, employees were working from a scissor lift at approximately 13 feet from ground level and approximately 29 inches from energized electrical lines. The table for this hazard indicates three feet as minimum safe distance. Employees at this location were exposed to possible electrocution hazards.
