29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): A.) The Tapco Tube Company Inc., 2nd floor high voltage room, Electrical Systems Box for the Welder - On or about 10/12/15, and at times prior thereto, a 480 volt electrical panel box was left unattended without the cover being placed back on the panel box. The room where the box was located what not locked or in anyway restricted from unqualified personnel entering the room while the cover was off of the panel box.

29 CFR 1910.335(b)(1): Safety signs, safety symbols, or accident prevention tags were not used where necessary to warn employees about electrical hazards which may endanger them: A.) The Tapco Tube Company Inc., 2nd floor high voltage room, Electrical Systems Box for the Welder - - On or about 10/12/15, and at times prior thereto, signage was not used to inform unqualified employees that the cover on the 480 volt box for the welder in the high voltage room was removed. The electrical hazards were created by a cover being left off of a 480 volt electrical box for the welder in the high voltage room. While the cover was off, the electrical parts inside the box were energized.

The electrical hazards were created by a cover being left off of a 480 volt electrical box for the welder in the high voltage room. While the cover was off, the electrical parts inside the box were energized.

The electrical high voltage room was located on a 2nd floor but the room was not locked or restricted through other equally effective means to prevent unqualified employees from entering it.
