29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Second Floor - Employees  standing on a 4 x 8 ft. sheet of plywood being handed up plywood decking were observed not protected from an 8 ft. fall hazard to the first floor, on or about 10/21/2105.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) Job Site - Employees performing work activity on the second floor and exposed to an 8 ft. fall hazard were not provided fall protection training , on or about 10/21/2015.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

No abatement certification or documentation required for this item.
