29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Second Floor - Employees standing on a 4 x 8 ft. sheet of plywood while being handed up plywood decking were observed not protected from an 8 ft. fall hazard to the first floor, on or about 10/21/2105. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) Lower Level at Existing Foundation Wall - Employees observed walking near several pieces of rebar protruding 4 to 12" were exposed to an impalement hazard, on or about 10/21/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
