29 CFR 1910.23(b)(1): 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded; On or about October 21, 2015, employees working on the second floor of the facility were exposed to a fall hazard due to the opening at the top of the staircase not being guarded to protect employees from falling to lower levels.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers were not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: On or about October 21, 2015, employees were using the staircase to access the second floor of the facility and the staircase did not have a handrail for employees use.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On or about October 21, 2015, employees were operating a Blue Giant BGL #0030115 powered pallet jack to transport boxes during shipping and receiving and the employees were not trained on the operations of the pallet jack.
