29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    (a)  Employees were installing siding from a ladder jack scaffolding and the employer did not ensure the employees were protected from falling to the lower level.  This exposed employees to a fall hazard of approximately 17 1/2 feet.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards.    (a)  The employer did not ensure that employees exposed to fall hazards had been trained to recognize and minimize the fall hazards associated with their work.

29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable:    (a)  Employees were installing siding from a ladder jack scaffolding  were exposed to fall hazards of approximately 17 1/2 feet and the employer did not ensure that employees were properly trained.
